Diabetes Education
The Diabetes Education Center at Jackson County Regional Health Center offers comprehensive diabetes services to people with type 1, type 2 and gestational diabetes. Our mission is to provide quality, personalized education to promote self-management skills and optimal health outcomes.
With a referral from a health care provider, participants receive diabetes instruction tailored to meet individual needs in the areas of:
- Blood sugar monitoring
- Medications and insulin
- Meal planning and nutrition
- Exercise planning
- Skin and foot care
- Goal setting
The JCRHC Diabetes Center is certified by the American Diabetes Association, Iowa Department of Public Health, and Iowa Medicaid Program. A team of registered nurses and a registered dietitian work together to provide clients with the skills they need to manage their diabetes. The team includes a Certified Diabetes Educator (CDE) who has comprehensive knowledge and experience in providing education for prediabetes, diabetes prevention and management.

Initial and follow-up diabetes education is available by a referral from your health care provider. The initial diabetes education self-management training sessions are designed to meet your specific needs and the recommendations of your health care provider.
Diabetes Self-Management Education includes:
- Healthy Eating
- Physical Activity
- Blood Glucose Monitoring
- Medication Management
- Risk Reduction
- Problem Solving
- Healthy Coping Skills
Follow-up diabetes education is available for those needing additional assistance in managing their diabetes. Follow-up education is recommended for anyone needing a change in their diabetes management plan, an education review, or additional training to improve diabetes management. The follow-up education is specific to individual needs with emphasis on problem solving to improve diabetes control.
People diagnosed with prediabetes are at a higher risk for developing type 2 diabetes and cardiovascular disease. Learn more about your risk for type 2 diabetes and ways to reverse it while developing healthy lifestyle habits. These classes are offered quarterly. Contact the diabetes education center for dates and cost.
Diabetes support group meetings are held monthly. These meetings are free of charge and provide a variety of education topics along with support to help manage your diabetes. Contact the diabetes education center for a calendar of meeting dates, times and topics.
This service is available by a referral from your health care provider.
A registered dietitian/nutritionist and certified diabetes educator is available for nutrition counseling for diabetes management, weight control, or any other nutrition concern. A referral from your healthcare provider is necessary for this service.
