Job Shadow Requirements

Observation Only:

• Completed application
• A copy of your immunization record
  • Influenza Vaccination within the last 12 months
  • MMR (titer or proof of two immunizations)
  • Varicella (titer or proof of two immunizations)
  • COVID Vaccination (if applicable)

Patient Contact:

• Completed application
• Mandatory Reporter Verification (Dependent Adult & Child)
• CPR Certification(s) – must be American Heart Association approved
• A copy of your immunization record
  • Influenza Vaccination within the last 12 months
  • MMR (titer or proof of two immunizations)
  • Varicella (titer or proof of two immunizations)
  • Tuberculosis Screening (Quantiferon Gold or T Spot preferred, but will accept 2 step TB Test within past 12 months)
  • COVID Vaccination (if applicable)

You must be currently enrolled in an applicable health sciences program in order to have patient contact. Programs that may be applicable are: Nursing, Radiology, Laboratory, and Physical/Occupational Therapy.
